摘要:Objetivo: Evaluar el impacto que el consultorio de medicina de familia tuvo sobre pacientes, alumnos y residentes. Identificar la influencia que un servicio de atención primaria ejerce en un complejo sanitario de atención primordialmente secundaria. Diseño: Investigación cualitativa. Material y Métodos: El consultorio funcionó durante 14 meses (Noviembre 2002-Diciembre 2003). Fueron atendidos 350 pacientes, por residentes, estudiantes de medicina, supervisados por un médico familiar. Resultados: Los resultados se dividieron de acuerdo con los tres grupos donde fue evaluado el impacto. Pacientes: buscan el nuevo servicio por insatisfacción con la atención del médico anterior. El 95% de los pacientes llegaron referidos por otros pacientes, por familiares, o por funcionarios del complejo sanitario. Residentes: aprendieron la práctica de la medicina centrada en el paciente, visualizando el poder terapéutico de la relación médico-paciente-familia. Estudiantes: mejoraron su conocimiento sobre la medicina familiar y encontraron motivación en sus estudios a partir del encuentro con el paciente. Conclusiones: Un consultorio de medicina familiar, integrado en un complejo sanitario de atención secundaria, es descubierto por los enfermos, y tiene resultados alentadores en la formación integral de residentes y de estudiantes, facilitando el despertar de futuras vocaciones para la medicina familiar
其他摘要:Objective: Our aim was to appraise the impact on patients, medical residents, and medical students when exposed to a Family Medicine out-patient physicians consulting office within a specialty care-focused public health facility without provision of primary care. Design: Qualitative study. Materials and Methods: In the period during which the project was run (14 months, November 2002December 2003), 350 patients were seen by medical residents and medical students and were supervised by a Family Physician. Results: In accordance with the groups in which impact was appraised, the results were collected into three groups: patients seeking a new medical service due to dissatisfaction with previous medical care, the majority of patients (95%) referred by relatives, other patients, or employees of the medical facility and not by physicians; medical residents, who learned about patient-centered medicine and who became aware of the therapeutic perspective of the physician-patient-family relationship, and medical students, who improved their understanding of Family Medicine and who found motivation for carrying out their academic tasks by means of the relationship they built with patients. Conclusions: A Family Medicine outpatient facility, even when incorporated into a health facility dedicated to delivery of secondary-level specialty medical care, is found by patients, possesses encouraging educational outcomes with medical residents and students and may promote vocations for future Family Physicians
