Studies of anterograde amnesia carried out during the acute phase showed that patients were affected in the first stage of episodic memory encoding. The episodic memory failure was associated to a dysfunction of the episodic buffer, a newly described component of working memory, which could account for the deficits in the integration of multimodal information. In the light of the current definitions of episodic memory, the memory disorder of TGA patients is regarded as an impairment of autonoetic consciousness. During the acute phase, conscious awareness is altered, which affects the ability to mental time travel from the past through the future. An extensive investigation of psychopathological factors have found that TGA comprises sudden changes in emotional state (high level of anxiety and a more depressed mood) that were no more found in the recovery phase. The question of the origin of these psychopathological disorders remains unsettled. In keeping with robust neuropsychological evidence, recent imaging studies finally suggest that an acute and reversible dysfunction (the cause of which is unknown) of the hippocampus, and possibly other parts of Papez’s circuit, could be the cause of amnesia in the TGA syndrome.
