摘要:Background: pure ankle dislocations are very rare injuries, they are caused by high-energy trauma especially traffic accidents and falls from heights. The treatment is usually surgical. Objective: to present a patient with traumatic, pure and open dislocation of the ankle joint. Case report: a 50-year-old woman, mixed race with no morbid health history, which suffers a height drop of approximately three meters and she is brought the Orthopedics and Traumatology service for presenting pain and total functional impotence of the right ankle. Through physical examination, open dislocation of the right ankle was detected with frank exposure of the articular surfaces of the distal tibia and the talus, as well as tendons and soft tissues. Taking into account all the above elements, it is decided to take the patient to the operating room, for surgical treatment. Immediately cleaning, pulsatile irrigation and debridement of the wound in the ankle joint, suture of the damaged soft tissue were performed; and external fixation type Hoffman® was placed in triangular type configuration. Conclusions: the treatment usually consists of the reduction and immobilization for six weeks, however in case of severe soft tissue involvement, where observation and wound healing is needed systematically, the use of external fixation is justified as the patient presented in this case.
